Charge of the Coordinated Entry / Discharge Planning Committee: Make recommendations to the Governing Board regarding discharge planning, ensure that current and appropriate agreements are in place with local institutions that may release individuals into a situation of homelessness, and work toward the outcome of successful release into a housed situation as opposed to homelessness.

The meeting will be held in person at the Social Services Shasta Conf Room located at 737 S. State St in Ukiah and will also be available via Zoom Teleconference.

Meetings are held every third Thursday until otherwise noticed, 9 am – 10:30 am
